Assigned to NRT, 11th Naval District at Long Beach CA 21 May 1960
Decommissioned 18 June 1960, placed "in service" as a unit of the Select Reserve ASW Force
Recommissioned 2 October 1961 for the Berlin Crisis, Decommissioned and placed "in service" 1 August 1962
Placed "Out of Service" 12 July 1969, Stricken 12 July 1969
Fate Sunk as target 18 February 1971 during testing of the Condor missile
