Flotilla 15 Group 44 Division 88

LCI (L) 1064 was commissioned on May 15, 1944 at USCG Station Bay City, Mich. Left Bay City, went through Lake Huron and Lake Michigan to Chicago, Ill., went down the Illinois River and Miss. River to New Orleans and then to Galveston and Corpus Christi, Tx. Left Texas for Panama Canal. From May 15, 1944 to June 27, 1944 we were at Bay City, Mich. and Texas.

We got in Panama July 3, 1944 and left Panama for Bora Bora for New Hebrides Is. Got there Aug. 4, 1944. Left New Hebrides Is. Aug. 8, 1944 for Admiralty Is., got there Aug. 15, 1944, Left there for the Lae and Hollandia Is. Stayed there from Aug. through Oct. 1944. Left Oct. 17, 1944 for the invasion on the Philippine Is. At Leyte Oct. 22,1944 made the invasion at Ormoc Dec. 7, 1944, Mindoro Dec. 15. 1944, Luzon Jan. 3, 1945, Panay Mar. 18, 1945, Negros Mar. 29, 1945 in the Philippines from Oct. 22, 1944 to Sept, 4, 1945 we were at the Philippine Is. Left Sept, 4, 1945 for Okinawa. Left Okinawa Sept. 26 for Shanghai, China, was in Shanghai, China from there we went to Formosa and Ninpo, China, Oct. 8, 1945, and to Formosa Oct. 25, was in China Sept. 29 to December 17, 1945, took the ship back to Pearl Harbor in March 1946 for U.S.A. It was decommissioned on June 6, 1946 and the ship was sold to Hovden Food Products Corp., P. 0. Box 391 Monterey, Calif., was stricken from Navy records Aug. 15, 1946.

The 1064 received the following medals: American Campaign Medal, Asiatic-Pacific (3 stars), World War II Victory Medal, Navy Occupation Service Medal, China Service Medal, Philippine Liberation Medal (2 stars), Philippine Presidential Unit Citation.
