ENCLOSURE "A" (Continued)

            A 1428. 12 April 1945, LSM(R) 189 while acting as close support ship or U.S.S. MANNERT L. ABELE (DD 733) in latitude 27@deg N and longitude 127@deg02 L LSM(R) 189 was attacked by enemy planes and succeeded in knocking down three and assisted in another. At 1502, one of these planes a KATE suicide dived the conning station and the following damage resulted:
        (a) Structural Damage
            1. Flag bag destroyed.
            2. All passageways and access to conn from superstructure deck demolished
            3. Flag bridge destroyed
            4. Port bulkhead in aft end of radio shack caved in and punctured
            5. Magnetic compasses in conn and wheel house knocked off stand and broken with stands wrenched loose from deck
            6. Chart table on conn destroyed
            7. CIC table in wheel-house destroyed
            8. Flood light on flag bridge destroyed
            9. Valve in fire main amid-ships, starboard side, broken
            10. Several small caliber shell holes from strafing, which did no appreciable damage
            11. Book shelves In chart room distorted
            12. Lights in chart room put out
            13. Rails and stanchions to hatchways bent out of shape
            14. Deck on flag bridge destroyed
            15. Jack box in wheel-house torn loose
            16. A section of electric light cable in chart room destroyed
            17. Electric light fixtures in chart room and passage-way to flag bridge ripped out

       (b) Material Damaged
            1. Barometer broken
            2. Thermometer - wet and dry bulb broken
            3. The following radios are out of order temporarily:
              1. One TDE transmitter
              2. One TCS transmitter
                  Ships's force may be able to repair then when antenna and wiring is replaced.
            4. Radar out
            5. Signal flag halyards destroyed
            6. Sound powered phones to after part of ship made inoperative
            7. Two Aldis Lamps damaged
            8. Sound powered megaphone on conn damaged
            9. ABK is smashed
            10. Two 6 ft. and two 10 ft. fog applicators destroyed
            11. Sixteen rockets launchers amid-ships damaged, dislodging ten rockets which were jettisoned
            12. Three radio antennas damaged materially
            13. Numerous radio and radar spare parts damaged
             14. Numerous flags and ensigns carried overboard
            15. Azimuth circle on gyro repeater and mount on conn sprung out of shape
             16. Numerous light bulbs through-out ship broken
             17. Two dummy polories on conn damaged
            19. Gasoline tank on fog generator sprung leak
